The following forms are available to allow for smooth processing of your membership application. All forms are available in Adobe Acrobat (pdf).

Form
 Description
Affiliate Form Use this form if you wish to join the CIOBS as an Affiliate member. All other grades of membership are obtained by completing the relevant qualification.
Reinstatement Form If you were previously a member of the CIOBS and wish to rejoin.
Fellows Nomination Form Guidelines for Fellowship nomination are available here
Direct Debit Form The annual collection of you subscription can be taken by diect debit (UK based members only). Please download this form to set up a direct debit.
